Referral for XRAYS

We accept referrals for Orthopantomograms (OPGs) and Lateral Cephalograms (Cephs) which we can email directly to the patient’s dental practice.

Dentists simply have to complete the following referral form, which can be signed and emailed directly to us, or given to the patient to bring to us. We will do the rest.

Referral for ITERO SCAN

Patients who require an iTero scan can be referred to The Oakwood Clinic. Our experienced Treatment Coordinators will take the scan and email the STL file to the patient’s dental practice.

Scans may be needed for the following reasons:
For patients who cannot tolerate impressions
For digital planning/restorative treatment
Implant treatment planning
For Invisalign refinements
For orthodontic superimpositions to identify tooth movements achieved
